Report criticizes failure to report small fire at nuclear weapons plant

A small fire occurred Dec. 20 just outside the main storage facility for highly enriched uranium at the Y-12 nuclear weapons plant, according to newly released information by the Defense Nuclear Facilities Safety Board and plant contractor B&W Y-12.
The biggest news wasn't the fire itself, which was reportedly extinguished in short order by the team working on a backup diesel generator and didn't hurt anybody or cause any real damage. The problem was the way the fire was reported at Y-12.
